CODE CM 671
TITLE Case Management A
UNIT 3
Prerequisite: Upper Division Standing
This is the first of the three-part Case Management course that offers a detailed inquiry into primary, secondary, and specialty care responsibilities in the clinical management of patients. Proper protocol for treatment planning, continuity of care, referral and collaborative efforts with other primary or secondary healthcare providers, as well as the need for follow-up care, determination of final review, and the selection of functional outcome measures will be included. Behavioral and psychosocial assessments, and the evaluation of patient's prognosis and future medical care will also be discussed. Case Management A, B, C courses do not have to be taken in sequence.
